Patient Registration

You can register as a new patient by completing our online form.

If you would like to register with the practice, please go here to ensure that you live within the practice area.

If you are unable to complete the online form, you can download a copy from this page. This can be emailed or returned to the practice in person.

It is important that you complete separate forms for each person who wishes to register, and we would be grateful if you could please inform us if you have any information or communication needs upon registration.

Please note, you will receive text confirmation when your registration has been processed, and you should expect to receive this within 1 week following submission of your registration form.

Proof of Identity

You are required to provide proof of identity when registering with the practice. This must include photographic ID and proof of address; examples of which can be found below.

Copies of these can be emailed to saville.med@nhs.net - please ensure you state your full name and date of birth when emailing your proof of ID to the practice.

Temporary Registration

If you are ill while you are away from home or if you are not registered with a doctor but need to see someone to receive emergency treatment from a local GP, you can register as a temporary patient.

You can be registered temporarily for up to 3 months. This process means that you will be temporary registered at our surgery but will also remain a permanent patient at your GP. After that time, you will have to re-register as a temporary patient or permanently register with the practice.

To register as a temporary patient, simply come into the surgery and fill in a temporary resident form. It will be processed as soon as possible and you will then be able to make an appointment or speak to a Clinician. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
